Guidelines required in creating the nomination should be a one page narrative (please: No lists or outlines) describing the purpose, conception, implementation, effort to date, and/or accomplishments of the nominee, and include:

	The full name of the project
	The creativity, purpose, variety, and perseverance despite obstacles and/or outcome of the peacemaking endeavor
	The name of the contact person within the group being nominated and the address and phone number (and/or e-mail address) where he can be reached
	The presbytery and the specific committee or entity of the presbytery that made the nomination
	The name of the peacemaking committee or entity within the presbytery and the name of its chairperson
	The name, address, e-mail, and phone number of a contact person (usually the person submitting the nomination) who may answer questions concerning the narrative and/or the nominee
	The name, address, e-mail, and phone number of  the person within the organization being nominated who will be listed as whom interested persons may contact for more information about the program or project
